Since Washington State is a "Direct Access" State, you do not need a doctor's referral to see a physical therapy at Vital Kinesis. However, we do recommend you to have a primary care physician to help manage your health care and a physician we could contact for medical purposes.
Yes — reimbursement is often possible! We recommend contacting your insurance company directly to confirm whether you have out-of-network (OON) benefits. To make this easier, we’ve included a list of suggested questions you can ask (see “How should I find out if my insurance has out-of-network benefits?”). If your plan does cover services from an OON provider, we can provide you with a detailed receipt (called a “superbill”) that you can submit to your insurance company for potential reimbursement.
Call your insurance company using the member services number (usually found on the back of your insurance card). Have the following list of questions ready to better understand your physical therapy coverage and potential out-of-network reimbursement.
1) What are my physical therapy benefits?
2) What is my out-of-network percentage of coverage?
3) Do I have a deductible? If so, how much is it?
4) How much of my deductible has been met?
5) Do I need a written referral from a physician? And does it have to come from my primary care physician?
6) Do I need authorization on file prior to starting physical therapy? If yes, is authorization on file already? Who should request the authorization?
7) Is there a special form I need in order to receive the reimbursement?
8) What is the mailing address for me to send the reimbursement form?
9) What is the maximum reimbursement rate (or 'allowed amount') for physical therapy services?10) Are there visit limits or caps for physical therapy under my OON benefits?
11) Will reimbursement be sent directly to me, or to the provider?
12) How long does reimbursement typically take once I submit a claim?
We accept payments in the form of cash, credit card, debit card and check. We also accept eligible Flexible Spending Account and Health Savings Account.
Please note that as of 7/1/2025, we now include a 2.6% surcharge fee, to cover the increased cost of credit card acceptance we must pay. Payments made with Zelle, cash or debit card won’t be surcharged. Thank you for your understanding
In the case where you have to cancel your appointment, we kindly ask that you provide at least 24 hours in advance for cancelation and/or rescheduling. If you cancel/reschedule with less than 24 hours of notice, we will be charging you for the cost of the full session. We do this because we want to be able to fill the spot with other patients and clients who may be waiting to get scheduled. You can cancel and/or reschedule via phone, text or email. Thank you for your understanding.
